5. * Important Information *

a. Supply Fee - A $30 supply fee will be imposed for each child enrolled to cover the cost of the books needed to ensure an efficient school year. If you need help covering the costs, please let the Sunday School office know in advance.

b. Meeting with Principal - Each family that fills out a registration form will be required to have a face to face or Facetime call on WhatsApp for a 20 minute meeting with the principal.

c. Monthly Fees - For one child, the total monthly fee will be $40. For two children, the total monthly fee will be $70. For three children, the total monthly fee will be $90. For four children, the total monthly fee will be $110. For five children, the total monthly fee will be $130. If you need help covering the costs, please let the Sunday School office know in advance.

d. Info. - If any of the information in this registration form is missing, incorrect, or not completed, your child(s) will not be registered into the Sunday School. If there are any problems, please contact Brother Youssef at the phone number of (484) 219-6791.
